All new patients must fill out New Patient Screening, Patient Information, Patient History, and HIPAA forms before your appointment; please bring those completed documents to your first appointment and plan to arrive 20 minutes early. Additionally, if you are currently enrolled in the Medicare program, please complete the Medicare and MEDIGAP forms as well.
